Multifocal pyomyositis after intra‐articular injections

چکیده
An 82yearold female who had received injections of hyaluronic acid into her knee joints several times because of osteoarthritis was admitted to a local hospital because of fever. Methicillinsusceptible Staphylococcus aureus was detected from cultures of blood and kneejoint fluid. Cefazolin administration was initiated. However, her condition did not improve for 3 weeks, so she was transferred to our hospital. She could not sit down because of systemic pain, but focal signs were absent. Computed tomography demonstrated multiple abscesses in trapezius (Figure 1A), triceps (B), iliopsoas (C), erector spinae (C), and quadriceps (D) muscles that extended around the hip joint and pubic bone. She was diagnosed with multifocal pyomyositis due to S. aureus. Management comprised surgical drainage and antibiotic administration for a long period. Pyomyositis is a pyogenic infection of the larger skeletal muscles and is usually accompanied by abscess formation. Pyomyositis is usually endemic in countries with tropical climates, but has been recognized with increasing frequency in temperate climates.1 Pathogenesis of pyomyositis is not known. Skeletal muscle is usually resistant to bacterial infection. Experimental injection of S. aureus into the leg muscles of dogs or intravenous injection of S. aureus does not result

Cushing's syndrome with suppression of the hypothalamic-pituitary-adrenal axis after intra-articular steroid injections.
A case of Cushing's syndrome after long term intra-articular corticosteroid injections is described. Prolonged suppression of the hypothalamic-pituitary-adrenal axis occurred as assessed by an insulin tolerance test 11 weeks after the last intra-articular injection. Replacement steroid therapy was required.
متن کاملLocal infiltration analgesia is not improved by postoperative intra-articular bolus injections for pain after total hip arthroplasty
BACKGROUND AND PURPOSE The effect of postoperative intra-articular bolus injections after total hip arthroplasty (THA) remains unclear. We tested the hypothesis that intra-articular bolus injections administered every 6 hours after surgery during the first 24 hours would significantly improve analgesia after THA. PATIENTS AND METHODS 80 patients undergoing THA received high-volume local infil...
متن کاملAcute knee joint rupture after yttrium 90 injection.
Two cases of acute rupture of the knee joint are reported after the intra-articular injection of radioactive yttrium 90. It is suggested that this may be a complication of intra-articular radioactive injections.
متن کامل[Multifocal pyomyositis after being hit by a truncheon in Gabon].
Pyomyositis is an acute primary bacterial infection of striated muscle. It affects mainly but not only immunocompromised patients, especially in tropical areas. The responsible microbe is often found to be Staphylococcus aureus. We report here the case of an immunocompetent 17-year-old with multifocal pyomyositis from which S. aureus was isolated two weeks after he was hit with a truncheon.
متن کاملPlacement of intra-articular injections verified by mini air-arthrography.
OBJECTIVE To develop and assess a simple, inexpensive method for ascertaining the placement of intra-articular injections for knee osteoarthritis METHODS During a one year period patients with "dry" osteoarthritis of the knee who received intra-articular therapy were tested by air-arthrography. Along with triamcinolone and lignocaine (lidocaine), 5 ml of air was injected into the joint. On su...
